Every individual or organization acting as a vendor (including independent contractors, presenters, or consultants) doing business with Sonoma County Office of Education (SCOE) is required to complete applicable state and federal tax forms. The following CA state tax forms can be found on the Forms and Publications page of the California Franchise Tax Board (FTB) website:
Required Forms:
Out-of-State vendors must submit a W9 and ONE of the following forms:
- Form 587* Nonresident Withholding Allocation Worksheet (For Out of State Vendors)
*Note: A Form 587 must be submitted for EVERY agreement - Form 590 Withholding Exemption Certificate (For Out of State Vendors)
Optional Forms for Out-of-State vendors:
- Form 588 Nonresident Withholding Waiver
- Form 589 Nonresident Withholding Waiver
- From 592-B Resident and Nonresident Withholding Tax Statement
